Parents night out

Drop your kids off Friday January 19th from 5:30-7:30p and enjoy some time to yourselves. There will be Four (4) adults on hand to watch the children and Pizza will be provided. All children must be potty trAined. proceeds BENEFIT FOCE. Space is limied, so act fast!

II/We hereby understand and acknowledge that the training, programs, and events held by Go! Kids' Gym may expose the participant to many inherent risks, including accidents, injury, illness, or even death. I/We assume all risks of injuries associated with participation including, but not limited to, falls, contact with other participants, and all other such risks being known and appreciated by the participant.

I/We herby acknowledge our responsibility in communicating any physical and psychological concerns that might conflict with participation in the activity. I/We acknowledge that the participant is physically fit and mentally capable of performing the physical activity he/she chooses to participate in.

After having read this waiver and knowing these facts, and in consideration of the acceptance of participation and Go! Kids' Gym furnishing services to me, I agree, for the participant and anyone entitled to act on their behalf, to HOLD HARMLESS, WAIVE AND RELEASE

Go! Kids' Gym, its officers, agents, employees, organizers representatives, and successors from any responsibility, liabilities, demands, or claims of any kind arising out of my participation in the Go! Kids' Gym training, programs, and/or events.

I/We indicate that I/We have read and understood the above Waiver of Liability. I/We are aware that this is a waiver and a release of liability and voluntarily agree to its terms. I/We have the legal capacity and authority to act on behalf of the minor named herein.
